Documentation isn’t admin paperwork.
It's risk management.
It's communication.
It's continuity.
It's client safety.

Why Communication Breaks Down in Practices

 Simple. Everyone thinks someone else is going to do it.  

Clinicians think admin will follow up.

Only one admin team member knows about it.  And they are away on holidays for 2 weeks.  

Owners assume the team knows the system.

Nobody wants to micromanage.

Everybody wants less email.

Nobody wants more meetings.

So practices become:

  • reactive
  • misaligned
  • reliant on memory
  • dependent on one person
  • constantly clarifying things twice, three times....no just kidding four times!
  • prone to conflict

1. Operational Communication

(admin ↔ clinicians)

2. Clinical-Adjacent Communication

(within scope)

Admin must know:

  • what to document
  • what not to document
  • where to place info
  • what to escalate
  • how to protect clinicians
  • how to protect clients
  • how to protect themselves

3. Team Communication

(owner ↔ team)

  • goals
  • boundaries
  • expectations
  • policies
  • feedback
  • changes

Clarity reduces conflict.
Structure reduces emotional labor.
Documentation reduces risk.

The Documentation Standard I Teach

1. Brief, clear, factual

Who needs to do it.
When they need to do it by.
Priority Rating.

2. Always logged in the same place

Not spread across:

  • email
  • text
  • PMS
  • sticky notes
  • personal phones

3. Consistent documentation structure

Try:

  • What happened [Context]
  • What we did [What happened, what did you try, why is this left for someone else?]
  • Action to take [What the client needs next, What the clinician needs to know, What needs follow-up]

4. Escalation rules are always clear

Admin should not guess.
Guessing leads to risk.

When Documentation Improves, Everything Improves

  • billing accuracy
  • client engagement
  • staff confidence
  • trust between admin + clinicians
  • onboarding new staff
  • risk management
  • time management
  • attendance

Documentation is not extra work.
Documentation is the work.
